ማሳሰቢያ- አባላት ይህንን ከአገር ውጭ ጉዞ ማስታወቂያ ቅጽ ሳይሞሉ ከ30 ቀናት በላይ ከዲኤምቪ (DMV) አካባቢ ውጭ የሚቆዩ ከሆነ አባላት ምንም ዓይነት ጥቅም አያገኙም።

NOTICE: Members who stay outside the DMV areas for more than 30 days without filling this travel departure form will not receive any benefits.

ለኅብረት መረዳጃ ማህበር አስተዳደር

እኔ የኅብረት መረዳጃ ማህበር አባል ይህንን የአባላት ከአገር ውጭ ጉዞ ማስታወቂያ ቅጽ ስፈርም የመተዳደሪያ ደንቡንና መመሪያዎች በማንበብና በመረዳት መብትና ግዴታየን በሚገባ አውቄ ነው። አባላት አገሪቱን ለቅቀው ከመውጣታቸው በፊት ይህን የጉዞ ማስታወቂያ ቅጽ እንዲያቀርቡ ይጠበቅባቸዋል።

For Hebret Mutual Aid Society, Inc. Board I agree to be bound by the Bylaws and Membership Manual of the Hebret Mutual Aid Society, Inc, having read and understood all the stated terms and conditions. Thereof all Hebret Mutual Aid Society members are required to submit the departure notification form prior leaving the country.
